Post-Hospital Rehabilitation
The rehabilitation area includes private rooms and a spacious therapy area for rehabilitation. Amenities include electric beds, nurse call systems, and rehabilitation professionals. These services are for residents who need specialized rehabilitation following a hospital stay. An intensive, short-term stay in the community is typically utilized prior to returning home to an active life. Seniors need a comfortable place to continue recovering and rehabilitating after completing a stay in the hospital. Short-term stays in the rehabilitation section of Grace Pointe help promote the transition back to home and a more independent lifestyle.
24-hour Staffing
Licensed nursing staff are provided for the residents of Grace Pointe, 24 hours a day, 7 days a week.
Rehabilitative Staff
Being able to rehabilitate after surgery or an illness in a way that helps residents return to home as quickly as possible is the goal of Grace Pointe. Licensed therapists and therapy assistants provide Physical, Occupational and Speech therapy at the doctor’s direction and in cooperation with nursing staff. We understand that everyone wants to live at home in the most dignified manner and carefully planned therapy will hasten the resident's return to home.
Medication Supervision
Medication needs of residents are directed through doctors’ orders that support each resident's care plan and health assessment. Grace Pointe staff have contact with physicians’ offices to address medication issues, changes in health status and emergencies.
Resident Health Assessments
Health Assessments are important for measuring preventative or appropriate care and quality of life. Our goal is to provide preventative care to support a resident's health, according to the specific needs of each resident. Assessments are completed while taking into consideration the resident’s health status and doctors’ orders. Meetings are planned with families to enhance communication and active participation regarding health conditions or resident need.
Assistance with Personal Care
To help residents look and feel good, staff can assist with the activities of daily living such as toileting, dressing, ambulation, bathing, nail care, shaving, hair care and make-up.
Grounds
Our courtyards provide privacy and security features for our residents. A doorbell is located in the foyer to alert staff after hours.
Full Service Salon
A salon is available to residents on-site so that services are much more easily accessible.
Housekeeping
Housekeeping services are provided for all rooms. Deep cleaning shall be completed routinely, while linen and bedding changes shall be done as often as needed.
Transportation
The handicapped-accessible bus will be available for scheduled doctor appointments as well as outings in the community that are appropriate for resident health status.
Meals and Snacks
Therapeutic meals will be served that take into consideration resident likes, dislikes, family suggestions and doctors’ orders.
Guest Meals
Family and friends are invited for meals, especially on holidays. Private dining will be available for large or small groups when scheduled with Grace Pointe.
Group and Individual Activity Programming, Fitness Programming
Grace Pointe provides a staff that assists residents with remaining physically and mentally active, while providing social interaction. The schedule will change and freshen on a weekly and monthly basis.
Counseling Options
Supportive staff can help families and residents throughout the transition of adjustment, and Grace Pointe offers a social worker on staff to assist families in finding the proper help and support during an emotional time.
