Memory Care
This aspect of our community manages residents who may need to be in an environment with security features to discourage wandering. The Memory Care area of the Grace Pointe community is designed with a comfortable residential atmosphere, including private rooms, kitchen, living room, dining room, and activities room. Private rooms offer a therapeutic, home-like atmosphere.
24-hour Staffing
Staff members are available to support the residents of Grace Pointe and to communicate with nurses in case of care needs, emergencies or change of condition. We have excellent proximity to North Colorado Medical Center that provides ambulance response and emergency services.
Resident Health Assessments
Health Assessments are important for measuring preventative or appropriate care and quality of life. Our goal is to provide preventative care to support a resident's health, depending on the specific needs of the resident. Assessments are completed while taking into consideration the resident’s health status and doctors’ orders. Meetings will be planned with families to enhance communication about health conditions or resident need.
Medication Supervision
Nurses participate in the medication needs of the resident by receiving doctors’ orders that support the care plan and overall health assessment. Grace Pointe staff will have contact with physicians’ offices to address medication needs and issues.
Assistance with Personal Care
To help residents look and feel good, our staff can assist with the basic activities of daily living. Some of these will include toileting, dressing, ambulation, bathing, nail care, shaving, hair care and make-up, just to name a few.
Specialized Memory Care Unit
Staff members will be trained in supporting residents as they progress with memory and dementia issues. Our activity program is designed to accentuate the things that residents can still do successfully. For additional information on our specialized Memory Care Unit, click here.
Grounds
Our courtyards provide privacy and security features for our residents. A doorbell is located in the foyer to alert staff after hours.
Full Service Salon
Residents are able to be scheduled for a full range of hair care services. We also will respond to residents’ requests to be taken to their familiar beautician/barber in town. Families may use our shop to obtain this service for themselves as well.
Housekeeping
Housekeeping services are provided for all rooms. Deep cleaning shall be completed routinely, and linen changes are scheduled as often as needed.
Transportation
Our handicapped-accessible bus can be available for scheduled doctor appointments as well as many outings, including trips to the mountains, local restaurants, shopping and visits to local city parks.
Meals and Snacks
Grace Pointe offers various breakfast options with flexibility for early or late risers. Lunch at mid-day is our main meal of the day, and a light dinner is served in the early evening. Residents are treated to visits from staff with the juice cart, and snacks are available any time. Meals are a social event, but we understand that illness may require a resident to have meal service in their room as needed.
Guest Meals
We love to include family and friends for meals, especially on holidays. Private dining will be available for large or small groups when scheduled with Grace Pointe.
Group and Individual Activity Programming, Fitness Programming
Grace Pointe's skilled staff assist residents with remaining physically and mentally active with social interaction. The schedule changes on a weekly and monthly basis. Please see our planned list of activities here.
Counseling Options
Supportive staff help families and residents throughout the transition of adjustment. Grace Pointe offers a social worker on staff to assist families in finding the proper help and support during an emotional time.
