Grace Pointe Continuing Care Senior Campus provides a special area that caters to assisted living residents who need help with the tasks of daily living, which may include bathing, dressing, medication reminders, and eating. Grace Pointe will have 36 private apartments where residents can share a lovely, spacious home environment that includes a dining area, activity and entertainment area, and a cozy sitting area around a fireplace.
24-hour Staffing
Staff members are available to support the residents of Grace Pointe and communicate with the nurse in case of care needs, emergencies or change of condition. We have excellent proximity to North Colorado Medical Center that provides ambulance response and emergency services.
Resident Health Assessments
Health Assessments are important for measuring preventative or appropriate care and quality of life. Our goal is to provide preventative care to support a resident's health, depending on the specific needs of the resident. Assessments are completed while taking into consideration the resident’s health status and doctors’ orders. Meetings will be planned with families to enhance communication about health conditions or resident need.
Medication Supervision
Nurses participate in the medication needs of the resident by receiving doctors’ orders that support the care plan and overall health assessment. We have contact with physicians’ offices to address medication needs and issues.
Assistance with Personal Care
To help our residents look and feel good, staff can assist with the basic activities of daily living. These include toileting, dressing, ambulation, bathing, nail care, shaving, hair care and make-up, just to name a few.
Grounds
Courtyards within the Grace Pointe community will provide privacy and security features for our residents. A doorbell is located in the foyer to alert staff after hours.
Full Service Salon
Residents can schedule a full range of hair care services. We also will respond to residents’ requests to be taken to their familiar beautician/barber in town. Families will be able to use our shop and receive these services as well.
Housekeeping
Housekeeping services are provided for all rooms. Deep cleaning shall be completed routinely, and linen changes are scheduled as often as needed.
Transportation
Our handicapped-accessible bus is available for scheduled doctor appointments as well as many outings, including trips to the mountains, local restaurants, shopping and visits to local city parks.
Meals and Snacks
Grace Pointe offers various breakfast options with flexibility for early or late risers. Lunch at mid-day is our main meal of the day, and a light dinner is served in the early evening. Residents will be treated to visits from staff with the juice cart, and snacks will be available any time. Meals are a social event, but we understand that illness may require a resident to have meal service in their room as needed.
Guest Meals
We love to include family and friends for meals, especially on holidays. Private dining will be available for large or small groups when scheduled with Grace Pointe.
