Dr. Marcus Reinhardt, Medical Director
Dr. Reinhardt was born in Kearney, Nebraska and moved to Greeley when he was two years old. He’s a 1990 graduate of University High School. He attended the University of Colorado for both his undergraduate and post-graduate Medical Degrees. After a residency at the University of North Carolina, he moved back to Greeley and joined Internal Medicine Consultants for 2 years, Internal Medicine at Greeley Medical Center for 3 years, and has been a hospitalist at NCMC for the last 4 years. With his wife Amy and son Levi, he spends his spare time camping, golfing, and reading.
Dr. Joseph Jacob, MD
Dr. Jacob is Board-Certified and is a specialist in Physical Medicine & Rehabilitation with Colorado Health Medical Group. He is a Director on the Board of North CO Medical Society and a delegate to CO Medical Society. Previously, he served as the Medical Director at Northern Colorado Rehabilitation Hospital for 7 years. He received his residency training at Temple University School of Medicine/ Moss Rehab Hospital in Philadelphia. Dr. Jacob and his wife Prema Jacob, MD, have 3 grown children. In his free time, Dr. Jacob enjoys church-related activities, reading, gadgetry and travel.
Deb Majors – Administrator
Deb has spent most of her life in the long term care environment. As a little girl, Deb remembers having lots of “grandpas and grandmas” around in the nursing home her parents owned. Deb worked several years as an Administrator, moving on to corporate management positions with Living Centers of America and Life Care Centers of America. Deb has a passion for the elderly, and enjoys Serving Others and making a difference in the lives of residents, families and staff, who come through the doors at Grace Pointe. In her spare time, Deb enjoys spending time with her family.
Rebeaka Varghese, Director of Nursing
Rebeaka was born in India, near New Delhi. She graduated in 1989, receiving her Bachelor’s of Nursing Degree from the University of Kerala, which is ranked 3rd in the top universities in India. She began her career in Cardiac ICU, but her true passion is geriatric care, and she’s spent the last 18 years in long term care. Rebeaka joined our Grace Pointe team before we opened our doors in the summer of 2009. She is married and has 3 children; following in her mom’s footsteps, her oldest daughter Liny is a Pre Med student at CSU. Despite Rebeaka’s busy schedule, she still finds time to tend to her beautiful flower gardens.
Madeleine Jacobs – Activity Director
Madeleine was born and raised in South-Africa. She’s been living in Vail, Colorado since 1996 and moved to Greeley in August 2012 to further her studies. She has a Bachelor in Business from the University of Bloemfontein in South-Africa and a Masters in Gerontology from the University of Northern Colorado. After working in a variety of positions including Hospice, CNA, hospitality and volunteering in Long term Care Facilities, it was clear that her true passion was leading her to service in Gerontology. In her leisure time she enjoys biking, nature, animals, exploring new places and her close relationship with God.
Steve Klady – Chef
Grace Pointe is fortunate to have Steve on staff: a Certified Chef and Certified Culinary Educator. Steve is a graduate and former employee of the Culinary Institute of America in Hyde Park, NY. He is a former member and Gold Medal Award Winner of the American Culinary Federation (ACF), and won a Colorado Chef’s Assoc. Award for the “Best Salad in the State.” He also owns a catering company called Tastefully Yours. In his spare time, Steve enjoys day trips, gardening, trying new restaurants, and enjoying dinners with friends. Chef Steve says it’s a “true honor” to Serve Others as Grace Pointe’s head chef.
Carol Fustos – Director of Rehabilitation, Physical Therapist
Born in Riverton, Wyoming, Carol lived in Pueblo, Colorado for 25 years. She was the Director of Rehabilitation at Parkview Medical Center for 18 years. She attended Casper College, on both academic and volleyball scholarships, and the University of Wyoming before graduating from the University of North Dakota with a degree in Physical Therapy. Carol’s special skills and interests include Neuro Rehab, cycling, traveling, gardening, scuba diving, and wine tasting.
Colleen Potter – Certified Occupational Therapist Assistant
Colleen is an assistant to our Occupational Therapist, working with treatment planning, to help our rehab residents achieve their goals. Colleen is a Colorado native, growing up in Brighton. She is a graduate of the Denver Institute of Technology. She also attended Heritage College in Denver for Massage Therapy. Her special skills and interests include hip replacement surgery rehab, soft tissue manipulation, outpatient and orthopedic rehab. Colleen enjoys spending time with her 4 year old daughter, cooking and baking.
Kathy Hammer – Registered Occupational Therapist
Kathy assists our rehab patients in achieving independence with their activities of daily living through strengthening, adaptation and modifications. Born and raised in Madison, Wisconsin, and a graduate of the University of Wisconsin, Kathy recently relocated to Colorado. She has many years of experience working with stroke victims, orthopedics and amputees. She loves working with the senior population, learning from them the value of tenacity, endurance and a hard work ethic. She finds joy in watching people “get better and return home.”
Cheri Moran – Physical Therapist Assistant
Cheri was born in Colorado and has been a PTA for over 17 years. After shadowing a Physical Therapist as a young girl, she knew she wanted to be a therapist. She worked in the NCMC Rehab for 15 years and also worked for a couple years in the school system. Cheri loves helping people no matter what age and prides herself in getting them back on their feet and functioning in society again.
Dan Olsen – Physical Therapist
Dan has lived all over, but calls Colorado home. He graduated with a Doctorate in Physical Therapy from Franklin Pierce University in Goodyear, Arizona. He is passionate about his job and enjoys helping people regain their independence and improve the quality of their lives. In his free time, Dan is an avid hiker, biker, and wood worker.
Katie Shoemaker – Physical Therapy AssistantRaised in Greeley, Katie graduated from Greeley Central High school and attended PTA school in Fort Morgan at Morgan Community College. Katie is also a licensed Massage Therapist. She has two daughters and lives in Eaton, CO. She enjoys hiking, horseback riding, being outdoors, and playing volleyball.
Amy Rizo- Physical Therapy Assistant
Amy was born and raised in Colorado. After graduating from the University of Northern Colorado with a bachelor’s degree in Sports and Exercise Science in 2008, Amy continued to follow her dream of becoming involved in Physical Therapy. Amy received her PTA degree from Morgan Community College in her hometown of Fort Morgan. Amy has a passion for her husband and daughter, working out, camping, fishing, playing softball or volleyball, and chocolate. She finds her job very rewarding as she meets the challenge of helping people return to their highest level of function.
Michael Northcutt – Registered Occupational Therapist
Born in Alton, Illinois and raised in Tucson, Arizona, Michael made Fort Collins his home in 1992. He graduated from CSU in 2000, and specializes in geriatrics. Michael enjoys hiking, mountain biking, and cooking. He also likes watching the Rockies play with his wife and three children. He enjoys helping others and assisting them in achieving their independence.
Elyse Lemoine – Speech Language Therapist
Born in Marshfield, Wisconsin, Elyse received her degree from the University of Wisconsin – Steven’s Point. Additionally, she received her Certificate of Clinical Competency in Speech Language Pathology, and has her Certificate in the McNeill Dysphasia Program. Elyse has worked in long-term care, sub-acute therapy, and enjoys working with traumatic brain injuries. She moved to Colorado in September of 2014. She loves hiking, listening to music, and watching movies.
Allison Moore – Registered Occupational Therapist Allison is originally from Neenah, Wisconsin and recently relocated to Fort Collins, Colorado. Allison received her Bachelor’s in Science in Rehabilitation Psychology at the University of Wisconsin-Madison, and has Masters in Occupational Therapy at Chatham University located in Pittsburgh, Pennsylvania. She has a passion for working with the geriatric population and enjoys her occupational therapy career. Allison enjoys exploring all that Colorado has to offer—hiking, mountain biking, skiing. She is an avid Wisconsin Badger and Green Bay Packer fan!
Teneile Lovelace – Certified Occupational Therapist Assistant
Teneile was born and raised in Goodland, Kansas. She is a graduate of Newman University in Wichita, Kansas with an Associates of Science Degree. As an Occupational Therapy Assistant, she helps patients attain their highest level of independence after an illness or injury. In her free time, Teneile enjoys bible study, fishing, long distance running and being with friends and family.
Megan Sellens – Speech Language Therapist
Originally from Fort Collins, CO, Megan received her Bachelor’s in Psychology from CSU. She was also a member of the women’s track and field team for the Rams, and received All Mountain West Conference scholar athlete honors for all 4 years. Megan then earned her Master’s degree in Speech-Language Pathology from UNC. She has her Certificate of Clinical Competency from the American Speech Language Hearing Association, and is certified in Vital Stim therapy for the treatment of dysphagia. She has been working in the skilled nursing setting for over 6 years. Megan and her husband currently have one son.
Lacy Vannorsdel – Speech Language Therapist
Lacy is originally from South Dakota, but has called Colorado home for the past 3 years. She received her Masters of Arts degree from the University of South Dakota in Speech-Language Pathology. She has a certificate of Clinical Competency from the American Speech Language Hearing Association and has worked in the school and medical settings. She loves snowboarding, rock climbing, yoga, camping and reading.
Dixie Hergenreter – Rehab Assistant
Dixie is a Colorado native and was raised in southern Colorado. She retired from the telephone company after 30 years then started working as a Rehab Tech and Rehab Assistant. Dixie has worked assisting program managers and therapist for over 10 years. She really enjoys helping the geriatric population. She and her husband have 3 sons and 7 grandchildren.
